If a patient with suspected airway obstruction becomes apneic while suctioning, what is the immediate action?

Begin ventilations with bag-valve mask and prepare for advanced airway.

When a patient with suspected airway obstruction becomes apneic, the priority is to restore ventilation immediately. Apnea means no effective air exchange, so providing oxygenation takes precedence over continuing suctioning or giving medications. Begin ventilations with a bag-valve mask to deliver high-flow oxygen and observe chest rise, using a two-person technique if possible for a better seal. As you initiate ventilation, prepare for an advanced airway so you can secure the airway quickly if ventilation remains inadequate or becomes difficult.

Continuing suctioning would delay ventilation and increase the risk of hypoxia during a critical moment. A rescue inhaler won’t resolve apnea and won’t restore ventilation. Calling for transport without addressing ventilation leaves the patient hypoxic, which is unacceptable in an emergency airway scenario.

Attempt another suction pass and observe.

Administer rescue inhaler.

Call for transport without intervention.
